Warlock invocation randomly fails

I seem to have a problem with my character Ismael. He randomly fails to cast his invocations. I have removed his armor and light shield and still the same problem. (he has the feat armored caster)

I now for a fact that this never happened when i start playing him. is there any other warlock that has this problem?

I have tried casting some invocations on my main and I do not seem to have any problems. The only time a spell failed for me was when I gave myself Spell Failure from polymorphing myself and instantly transforming back, and when Flee the Scene didn't apply correctly, but that was a one-time thing I noticed.
Are there any specific spells that are failing for you, or is it all of them? Because for me there is no real problem.

If you are equipping items then trying to cast it will likely fail.
When you equip a weapon it induces arcane spell fail.

Yes, I've figured out that after you polymorph back to your regular self from Word of Changing, the next spell you cast is auto-fail, no matter how long you wait. Don't know if resting interferes with it though.

Do you get a print to your combat log that says Cannot cast spells while polymorphed when this happens or does the spell fail w/o feedback?

No feedback. The vfx of the casting happen (hands light up and stuff) and then nothing. There is no message that the spell did not complete or failed, just nothing happens.

Ditto. Auto-fail, no feedback, next spell fails no matter how long after Word of Changing.

It is kinda like that scene in Something About Mary, you look all around because you were pretty sure it went off, only no one ever finds the completed spell hangin off your ear and uses it for hair gel. I think my analogy went of track somewhere, but I am sticking with it.
